This is a fine list, full of insight. The "tells" you chose are very much to the point IMO, especially 4 and 7. Other points might be added to the list, such as how you act when no one's watching (e.g. do you return the shopping cart) and how you take care of your living space, but this is certainly a good start.

One thing you said caught my attention, being at odds with my experience. Perhaps it was because of the settings where I practiced, but most of my clients were not drawn to therapy by a hunger for self-discovery. They wanted life to hurt less, and other than a blessed few, they wanted no more insight than was absolutely necessary when all other options were exhausted.
