Managed care may have been a good idea at some point. It could be done well now. Pay the company to keep you healthy, give them incentives to load up on prevention and early detection/intervention, with the proviso that they will take proper care of you when sick or hurt.
But capitalism simply cannot come up with a working model for health coverage, because the interests of the shareholders will always be competing with the interests of the clients.
I fought many battles with managed care when I was in private practice, back in the bad old days before Obamacare made them stop putting caps on coverage and refusing coverage for preexisting conditions. Dollars versus lives and human suffering. Disgraceful.
You know what you need, so keep on squawking till you get it.
